   Additionally, for drugs considered maintenance medications, you can obtain up to a 90-day supply through the plan's mail-order program.  If you obtain a 30-day supply you will be responsible for one co-pay.  If you obtain a 60-day supply you will be responsible for two co-pays.  If you obtain a 90-day supply you will be responsible for two-co-pays, a savings of one co-pay.

   You must obtain a prescription from your physician stating the exact number of days being requested.  The mail order program cannot fill a script without prior approval from your physician.

    A mail Order Form and original script must be mailed directly to the mail order vendor.  Forms can be obtain through the Plan Office or online at AmeriPharm.
